Scuba diving can be physically challenging and a medical examination may be required
before starting a course. A Medical statement must be completed prior to any course.

In addition to the medical statement you are required to complete and sign the
Safe Diving Practices Statement and Liability Release & Assumption Of Risk Agreement
before commencing any PADI program.
Certified divers must complete the Liability Release & Assumption Of Risk Agreement.
